The Olympic Council of Ireland is gathering accreditation and clothing information from athletes and support personnel who have any possibility of attending the European Youth Olympic Festival in Tbilisi, Georgia this summer and they are asking all athletes to complete a questionnaire ASAP.

Please note that the completion of this questionnaire is purely for administrative purposes and in no way signifies selection as a member of the Irish team. All potential team members should complete the questionnaire at this link EYOF Advance Data Capture as soon as possible, but before Wednesday 29th April at the latest. 

Required Information

Failure to do so may result in accreditation not being processed in time, & could also result in the respondent not receiving the correctly sized team uniform.

Prior to completing the questionnaire the potential team member will require soft copies of the following items ready to upload with the details that they will also type in:

• The details page of their passport page
• A head and shoulders photograph of passport standard
