Athletes born in 2006 or 2007

The Olympic Federation of Ireland (OFI) is gathering accreditation and clothing information from athletes and support personnel who have a possibility of participating in the European Youth Olympic Festival (EYOF) in Banská Bystrica, Slovakia as part of the Irish team. Please be aware that the completion of this questionnaire is purely for administrative purposes at this stage and in no way signifies your selection as a member of the Irish team.

Should you be selected as part of the Irish team at EYOF, you may be required to complete some further questionnaires and other documentation.

You should also be aware that some of this information will be passed to the Organising Committee as part of the accreditation process.

To complete this data capture you will need a scanned copy (or photograph) of your passport details page and also a digital headshot (you can use your phone camera). Support Personnel will also require a valid Garda Vetting certificate.
