To Potential EYOF 2019 Team Members (Only athletes born in 2002 or 2003)

The Olympic Federation of Ireland is gathering accreditation and clothing information from athletes and support personnel who have a possibility of travelling to the European Youth Olympic Festival in Baku, Azerbaijan as part of the Irish team. Please be aware that the completion of this questionnaire is purely for administrative purposes at this stage and in no way signifies your selection as a member of the Irish team.

Before starting you should scan or photograph your passport details page and a head & shoulders 'passport quality' photo and save them on the device you are using to complete the form.
The photo does not have to be professionally taken. A well taken, in focus, photo using with a digital camera or phone camera will suffice.
Photograph criteria;
• Digital image file (.jpeg, .jpg, .png, .gif or .bmp)
• Plain white Background
• Mouth closed, Neutral Expression, looking forward
• 100Kb-1Mb size
